on the farmBluegrass Community Health Center (BCHC) is dedicated to providing comprehensive primary healthcare utilizing an integrated health model inclusive of Behavior Health services. BCHC was originally established in 2001, with a federal grant to serve local farm workers. In 2007, additional grant funding allowed an expansion to serve persons experiencing homelessness. BCHC serves over 8,000 patients in an eight county service area including: Fayette, Woodford, Jessamine, Garrard, Madison, Clark, Bourbon and Scott Counties. BCHC is currently welcoming new patients and accepts Medicaid, Medicare, Commercial Insurances and serves the uninsured with a fee based on income.

As many of Kentucky’s migrant and seasonal farm workers are Spanish speakers, BCHC employees bilingual staff who speak English and Spanish and utilizes language line interpretation to ensure that all patients have access to clinic services in any language. BCHC serves the migrant farm worker and the homeless in clinic and with outreach services inclusive of a medical team going to various locations to provide access to care.

Bluegrass Community Health Center utilizes the Integrated Care Model in which behavioral health services are offered in the primary care setting. Behavioral health services include lifestyle support for chronic disease management and outpatient counseling for mental health and substance use needs. The behavioral health providers work in conjunction with the primary care provider to evaluate the mind-body-behavior connection and provide brief, solution-focused interventions to support improved health outcomes.
